We built a HIPAA-compliant application that a Nebraska foster care and family services organization now uses to manage its daily service documentation.

The organization had outgrown its paper-based process
The organization provides foster care and family services under contract with the Nebraska Department of Health and Human Services.
Each visit required staff to complete Daily Service Notes, Travel Logs, and other supporting documentation. Much of that information was recorded by hand in the field and then entered again into additional forms or systems once staff returned to the office.
The process created several challenges.
Staff frequently entered the same names, service details, travel information, and notes more than once. This took time away from working directly with children and families.
Documentation errors also carried more weight than a typical clerical mistake. Incomplete or inconsistent records could create compliance concerns and affect the organization’s state reporting.
Supervisors had limited visibility into work happening in the field. They often could not confirm which services had been completed or which documentation was still missing until the paperwork reached the office.
At the same time, every part of the process had to meet strict privacy and HIPAA requirements.
The paper system had worked when the organization was smaller, but it had become increasingly difficult to manage as the number of services and employees grew.
Building one system for field documentation
We developed an application that allows staff to enter service information once and use that information throughout the documentation and reporting process.
Digital service notes and travel logs
Staff previously completed Daily Service Notes and Travel Logs by hand and often re-entered the same information later.
They can now document services directly from a phone or tablet while working in the field. This reduces duplicate entry and helps ensure records are completed closer to the time the service occurs.

Preloaded participant information
Employees regularly work with the same children, family members, and other participants. Instead of typing the same names and information for every visit, staff can select participants who are already associated with the case.
This saves time and reduces inconsistencies caused by repeatedly entering the same information.
Automatic travel calculations
Travel time and mileage were previously calculated by hand.
The application now calculates this information using the start and end details entered by the employee, reducing manual calculations and making travel records more consistent.

Better visibility for supervisors
Supervisors no longer have to wait for paperwork to return to the office before reviewing documentation.
They can see which services have been documented, identify missing information, and follow up while the details are still recent. The system also sends reminders when required documentation is overdue.

HIPAA-compliant documentation
The application was designed around the organization’s privacy, security, and documentation requirements.
Instead of relying on the handling and storage of physical paperwork, service records are managed through a centralized system built to support accurate and secure documentation.
The results
The organization now tracks thousands of services each month without adding administrative staff to manage the additional workload.
Employees spend less time completing repetitive paperwork, while supervisors have a more current view of the services being provided and the documentation that still requires attention.
The system has also helped reduce reporting errors by making required information easier to complete, review, and manage.

Where they are now
Documentation is no longer something the organization can review only after paperwork reaches the office.
Staff can record services from the field, supervisors can identify missing information sooner, and leadership has better visibility into activity across the organization.
As the organization serves more children and families, the application gives it a more reliable way to manage documentation without increasing the administrative burden at the same rate.
